effort to improve the health of our community, we are conducting a survey
of the environmental health practices and concerns in Chippewa County.
For us to provide the best service possible, we need your input.
Help us determine what really matters. We invite you to take part in this
survey regarding several areas of community environmental health. No
personal data is collected that can identify the individual. All data
collected is used only for statistical purposes of the project.
The survey should take 15 minutes of less to complete.
